Welcome to Pure Life Counseling. By using our website, client portal, or counseling services, you agree to the following Terms and Conditions. Please read them carefully.

1. Services Provided

• We provide individual, couples, family, group therapy, Christian counseling, and psychological evaluations (including immigration evaluations).

• Services are offered in-person or online through secure telehealth.

• Our services are not for emergencies. If you are in crisis, call 911 or go to the nearest emergency room.

2. Appointments & Scheduling

• Appointments are scheduled through our office or via the SimplePractice Client Portal.

• You are responsible for providing accurate information when scheduling.

• You are responsible for keeping your portal login secure.

3. Telehealth Services

• Telehealth is provided through SimplePractice’s HIPAA-secure platform.

• You are responsible for having a private, safe space for sessions.

• Technical issues may occur; we will work with you to reschedule if needed.

4. Fees & Payments

• Fees are discussed during intake.

• We accept insurance plans and self-pay.

• Clients are responsible for costs not covered by insurance.

• Payment is due at time of service unless otherwise arranged.

5. Cancellations & No-Shows

• Please cancel or reschedule at least 24 hours in advance.

• Late cancellations or missed appointments may result in a cancellation fee.

6. Confidentiality

• Your information is protected under HIPAA.

• Exceptions: if there is risk of harm to self/others, suspicion of abuse, or if required by law.

7. Client Responsibilities

When using our services, you agree to:

• Provide accurate personal and health information

• Use services only for appropriate purposes

• Treat staff and others with respect

8. Service Limitations

• Counseling outcomes are unique to each client and cannot be guaranteed.

• Therapy is not a substitute for medical treatment. Please continue any prescribed medical care.

9. Website & Client Portal Use

• Our website and portal are for personal use only.

• You must not misuse or attempt to gain unauthorized access.

• The SimplePractice Client Portal has its own Terms of Service that you must also follow when scheduling or attending sessions online.

10. Privacy

Your use of our services is also subject to our Privacy Policy, which explains how we collect and protect your information, as well as SimplePractice’s Privacy Policy for client portal use.

11. Legal Jurisdiction

These Terms are governed by the laws of the Commonwealth of Pennsylvania, United States.
